**Please note that although we are able to electronically bill most of the extended plans listed below, some plans have a deductible, some only allow payment to be issued to you the member and not to us the provider, and some plans do not allow electronic submissions at all.  Also, some plans require a doctors referral which we are unable to anticipate.
